Viking Stadium Rules	These rules are intended to be printed out or e-mailed to your team and to your opponents so our club can follow the guidelines and rules established by the school district for use of the stadium.
Wembley Soccer Academy	Use this form to register for Wembley Soccer Academy's excellent summer camp to be held at Evergreen Primary this July/August.

Forms for Coaches

Form Title	Description
2007 UPSC Team Affiliation Form	Please use this form to affiliate your team to play soccer in UP.
2007 District III Affiliation Form	The 2007 Team Entry Form is now available online. All teams must submit their District III Team Affiliation Form to UPSC prior to July 15th.
2007 TPCJSA Affiliation Form	These forms must be completed online, but they are not yet available. Please visit the TPCJSA website prior to July 15th to complete this form. All teams must submit their online TPCJSA Team Affiliation Form prior to July 15th.
Guest Player Form	Use this form to allow a player to participate as a guest player on another team for non-league or non-cup play with the permission of the player's coach.
