2011 UPSC Uniform Order Guideline

The following guideline applies to all teams U-9 and above only. Coaches are to coordinate orders and collect payments and size information from parents.

U-6, U-7, and U-8 MOD teams will have uniforms provided as part of their registration fee. MOD Coaches will receive uniforms prior to the first game of the season.

Ordering for U-9 and above:

• Each team shall designate one representative to submit uniform orders.

• Designated representatives may e-mail uniform questions to David Hunter at: dshunter2@comcast.net. Please reference contact name, team name, age division, and include a contact phone number.

• A blank order form is attached. Order forms can be faxed upon request. Use the order form for apparel only. See below for equipment orders.

• Completed order forms must be e-mailed back to dshunter2@comcast.net starting April 11, 2011 but no later than August 10, 2011. Orders received after August 10 may not be available prior to the first game of the season.

• Once an order is received, the sender will receive a confirmation e-mail.

• No payment or deposit is required when placing an order. However, prior to delivery, orders must be paid in full with a single check. A team name must be referenced on the check. Mail checks made payable to "UPSC" to: UPSC, c/o David Hunter, 3202 Tyee Dr. W, UP, WA 98466.

• The team representative will be contacted when uniforms are ready for pick up.

The standard UPSC uniform kit (Jersey, shorts, and socks) is the DTI Soccer "Roma" style - this is the same uniform kit as last year. DTI offers this kit with a Royal Blue or White jersey as shown below. Choose one color or both for home and away jerseys. Shorts and socks are royal blue.

Uniform Ordering Form

2011 uniform prices are as follows and include Washington State Sales Tax:

• Youth size kits - $22.00 each;

• Youth size jersey only - $14.00 each;

• Youth size shorts only - $7.00 each;

• Adult size kits - $25.00 each;

• Adult size jersey only - $16.00 each;

• Adult size shorts only - $8.00 each;

• All socks - $3.00 each.

Price includes a number on the back of the jersey and a UPSC crest on the left chest. Note: Any kit with an Adult size jersey defaults to Adult kit pricing.

UPSC pricing also includes most items on the DTI website (www.dtisoccer.com), i.e. coaching equipment, balls, warm-ups, goalkeeper uniforms, etc. These items are offered at the next highest volume discount as shown on the DTI website. For example, the price for quantities of 1 to 9 DTI "Panther" soccer balls is $9.95 each, plus tax. Under UPSC pricing, the same ball is available at the 10 to 49 quantity price of $8.95, plus tax for quantities of less than ten, a $1.00 savings per ball.

Equipment orders can be submitted as an e-mail and do not require a separate order form.
